Oaks Medical Centre

In order to register with the practice we ask that you call into the surgery, preferably between 10.00am and 3.00pm. A receptionist will take some details and make an appointment for you to attend for a New Patient Check. Please bring along your Medical Card if possible and/or your NHS number. We will also require proof of identity (eg passport) and address (eg recent utility bill).
